Review only the final delta from d4af978 to this pinned commit, described in docs/review-credit-fixes.patch and docs/review-credit-findings.json. Original paid review job5e2ee286-9298-4fa2-a0d0-5c4b387c7dc8 already examined the prior native prepaid route and finite caps; do not repeat its entire audit.

Verify the corrections: agent requires surplus_credit for fresh dispatch; exact-ID explicit unsigned pre-reservation capacity deferrals queue the same ID; inactive uncertain delivery after the original800-second window fails the agent review without releasing monetary holds, approving, or retrying the paid call; final output/input quote cannot shrink; readiness honors configured ceiling; dated canonical-decimal capacity is bound to at most two configured project IDs, all other projects stay at8.

Look for concrete introduced replay/accounting/authority defects. Read full changed mirrors plus now-included unchanged vendor quote/response/policy and completion-stream. Existing ledgers, wallets, contracts, model/effort, budgets and histories are unchanged.

Deployment manifests explicitly distinguish historical verified source from unpublished candidate. No contract or customer-fund changes. Targeted tests and typecheck passed.

Report material defects separately from documentation/style and clearly distinguish unchanged risks. This is the final corrective delta, not a request to expand scope or repeat unchanged suites.

  • 1.mediumOnly credit_busy carries admission:'deferred'; every other exact-ID unsigned pre-reservation refusal turns the review uncertain, then terminally fails it as inference_not_reservedintegration/payment-review/src/index.js:157

          if(!admission.ready)return c.json({id,error:admission.reason,payment:'not_signed',...(admission.reason==='credit_busy'?{admission:'deferred'}:{})},503);

MATERIAL, introduced by this delta (liveness/attempt accounting; no customer funds move, no monetary hold is taken on this path). The 'Admission race' correction says exact-ID unsigned pre-reservation busy/capacity deferrals requeue the same agent row. The payer stamps every POST-time workAdmission refusal with the exact id and payment:'not_signed', but attaches admission:'deferred' only for credit_busy.

The agent (integration/agent-review/network-review.ts:120-125) requeues only when admission==='deferred' and error is credit_busy or credit_capacity_wait; everything else falls to if(!response.ok)throw and the catch at line 132 sets state='uncertain'. credit_exhausted is the same capacity condition observed one check earlier: credit-work-admission.js:18 tests remaining<2000000 before the busy test at line 19, so a concurrent Builder reservation that lands between the agent's admission GET and its POST while the lifetime budget is within 4 USDC of its ceiling produces credit_exhausted, not credit_busy. credit_unavailable (claus_credit_work_control.frozen flipped by a Builder overrun) is equally transient and unsigned.

For all of these the review holds the single claus_network_review_active slot for 800 s, networkReviewStatus then calls GET /api/requests/, which finds no row and answers 404 not_found via confirmAbsent, and lines 159-161 set state='failed' {error:'inference_not_reserved'}.

Because the review id is nr_+hash(input) and submitNetworkReview returns the existing row (lines 61-62), the identical proposal evidence can never be re-reviewed, and one of the planned 'at most two further decisions' and one of the day's eight admissions is consumed by a request that never reached a quote. Merged from audit_economics 8f29ceca, audit_math fd776c64 (variant a), audit_flow c247d32d and audit_permissions ce215781.

Minimal fix: in index.js:157 emit admission:'deferred' for every pre-reservation workAdmission refusal (they are all unsigned and pre-reservation by construction), or at least for the transient set {credit_busy, credit_exhausted, credit_unavailable}; alternatively have the agent fail immediately, rather than go uncertain, on any payer body with id===effectId, payment==='not_signed' and no admission marker.

Post-reservation 502 payment_requires_reconciliation responses and transport errors must keep their current uncertain handling.

    State: NETWORK_REVIEW_FRESH_RAIL=credit_work, SURPLUS_CREDIT_ENABLED=true, pool funded, claus_payment_budget ceiling=100000000 committed=97000000 (remaining 3000000), no active claus_credit_work row, agent row nr_X queued.

    1. Agent GET /api/admission/network-review:nr_X?maximumMicroUsdc=2000000 -> workAdmission: remaining 3000000>=2000000, busy=false -> {ready:true,rail:'surplus_credit'}; agent claims the row (state='running', started_at=now).
    2. Before the agent's POST, a Builder POST with Idempotency-Key build_<24hex>:model:1 reserves 2000000 (committed=99000000) and completes within the same second (row inactive, busy=false).
    3. Agent POST /api/completion Idempotency-Key network-review:nr_X -> index.js:156 workAdmission -> credit-work-admission.js:18 remaining=1000000<2000000 -> waiting('credit_exhausted') -> line 157 responds 503 {id:'network-review:nr_X',error:'credit_exhausted',payment:'not_signed'} with no admission field. EXPECTED (docs/review-credit-routing.md line 9, docs/review-credit-findings.json 'Admission race'): the agent returns the row to state='queued' with available_at=now()+60s, exactly as it does for credit_busy. ACTUAL: network-review.ts:120 is false, line 125 throws, line 132 sets state='uncertain'; after 800 s networkReviewStatus receives 404 {error:'not_found'} and sets state='failed' {error:'inference_not_reserved'}; submitNetworkReview for the same input returns that failed row forever. Executed: the unmodified network-review.ts (sha256 93a9462b...) was run under Node 24.21 with a stubbed db and fetcher (admission GET -> ready/surplus_credit; POST -> the 503 bodies above). Results: credit_busy and credit_capacity_wait deferrals -> 'queued' (controls pass); credit_exhausted, credit_unavailable, credit_ceiling_mismatch, credit_not_configured -> 'uncertain', then after a status call at started_at+801 s with payer 404 not_found -> 'failed' {error:'inference_not_reserved'}.
  • 2.lowOverlapping reserveCredit statements: the loser's 23505 on claus_credit_work_one_active is uncaught and surfaces as a bare 502 without id, so the genuine admission race is not deferredintegration/payment-review/src/credit-provider.js:65

        const reserved=await store.reserveCredit(db,{id,hash,maximum,activeMs:timing.activeMs,binding:{address:pool.deposit_address,keyId:pool.key_id,transaction:pool.deposit_tx,

MATERIAL but rare, introduced gap in the 'Admission race' correction (liveness only; the losing statement rolls back entirely, so no hold or ID claim is left behind). allowCapacityDeferral only converts an EMPTY reserveCredit result (the admitted CTE matched nothing because the other reservation was already visible) into {id,error:'credit_capacity_wait',admission:'deferred',payment:'not_signed'}.

When the agent's and a Builder's reserveCredit statements actually overlap, the second waits on the claus_payment_budget row lock, re-evaluates committed BETWEEN ... on the new row version but keeps its original snapshot for the NOT EXISTS(SELECT 1 FROM claus_credit_work WHERE active) subquery, passes admission and then violates the partial unique index claus_credit_work_one_active (credit-work-store.js:21; its own comment at lines 54-55 describes exactly this).

The resulting 23505 is thrown from line 65, which is outside the try that starts at line 79, so creditCompletion rejects; completion-stream.js:13-16 logs completion_handler_failed and returns failure() = 502 {error:'payment_requires_reconciliation'} with no id and no deferral marker. The agent then goes uncertain and, after 800 s, fails terminally with inference_not_reserved as in the previous finding.

Distinct mechanism and fix location from the index.js:157 finding (audit_math fd776c64 variant b).

Minimal fix: wrap store.reserveCredit in try/catch and, when allowCapacityDeferral is set and error.code==='23505' with constraint 'claus_credit_work_one_active', return the same 503 credit_capacity_wait deferral; any other 23505 (for example claus_payment_ids_pkey) must keep the current non-deferral behaviour because that ID is already claimed or fenced.

State: as in the previous finding but with remaining>=4000000 and no active work row.

Run a Builder POST (build_<24hex>:model:1) and the agent POST (network-review:nr_X) so that both pass workAdmission (busy=false) and their reserveCredit statements execute concurrently.

EXPECTED (docs/review-credit-findings.json 'Admission race'): the loser answers with an exact-ID unsigned deferral and the agent requeues nr_X.

ACTUAL: the loser's statement raises 23505 on claus_credit_work_one_active; credit-provider.js:65 propagates it; streamCompletion returns 502 {"error":"payment_requires_reconciliation"}; the agent marks nr_X 'uncertain' and after 800 s 'failed' {error:'inference_not_reserved'}.

Executed: completion-stream.js run verbatim under Node 24.21 with a handler that rejects with {code:'23505',constraint:'claus_credit_work_one_active'} produced status 502 body {"error":"payment_requires_reconciliation"}; feeding that body to the unmodified network-review.ts runNetworkReview produced state 'uncertain', and a status call at started_at+801 s with payer 404 produced 'failed' {error:'inference_not_reserved'}.

  • 3.infoDocumentation: the elevated recovery limit is a global-day ceiling compared against all projects' reviews, not a per-project allowance; 'all others retain eight' overstates itintegration/agent-review/network-review.ts:66

        WHERE (SELECT count(*) FROM claus_network_reviews WHERE created_at>=date_trunc('day',now() AT TIME ZONE 'UTC') AT TIME ZONE 'UTC')<${networkReviewDailyLimit(Date.now(),process.env,input.projectId)}

Documentation/style, not a replay, accounting or authority defect. networkReviewDailyLimit returns 8..16 only for an allow-listed projectId (verified: allowlisted -> 10, other project -> 8, no projectId -> 8, limit '010' -> 8, three IDs -> 8, wrong UTC day -> 8), but the operand it is compared against is count(*) of ALL reviews created today.

The allow-listed project's extra attempts share one counter with everyone else, so an allow-listed project can consume the whole day and leave other projects with zero, and ordinary morning submissions reduce the allow-listed project's recovery headroom.

This mirrors the network side (src/approvals/limits.mjs:30 compares requestedGlobalDay against the shared global-day counter), so the tiers are consistent and nothing resets, but docs/review-credit-routing.md line 5 and docs/operations.md line 128 say other projects 'retain eight', which reads as a guaranteed allowance. Merged from audit_economics 279b8b72 and audit_permissions d7ad8b71.

Fix: reword the docs to say eight is the ceiling for non-allow-listed projects and capacity is shared, or scope the extra headroom per project (global_count<8 OR project_count_today<limit-8).

    Env: NETWORK_REVIEW_RECOVERY_PROJECTS=prj_45b80d1b-453d-4ebc-a7bd-e15098e6c5fb, NETWORK_REVIEW_RECOVERY_LIMIT=16, NETWORK_REVIEW_RECOVERY_DAY=.

The allow-listed project submits 16 distinct valid proposals (count 0..15 < 16, all inserted). prj_6a2a275a-abc7-4e29-b91e-4e004ebb58ef then submits its first review of the day: limit=8, count(*)=16, 16<8 false -> INSERT returns no row -> submitNetworkReview throws network_review_capacity.

Expected per the docs' wording: admitted, since that project used none of its eight.

Actual: refused for the rest of the UTC day. networkReviewDailyLimit values above were obtained by calling the exported function from the unmodified file under Node 24.21.

  • 4.infoUnchanged risk: terminal failure of an inactive uncertain delivery frees the agent slot while each 2 USDC work-store hold is never released, so unresolved exposure accumulates toward the lifetime ceilintegration/payment-review/src/credit-work-store.js:45

RETURNING CASE WHEN accounted_cost=0 AND dispatched_at IS NULL THEN reserved_max ELSE 0 END AS released

Not a defect of this delta; this is the documented design (credit-work-store.js lines 1-4 and docs/review-credit-routing.md line 9 'the payer ledger keeps the full unresolved exposure'), recorded because the task asks to distinguish unchanged risks. The new branch at network-review.ts:153-157 correctly marks the review failed with paymentHoldRetained:true and never approves, re-POSTs or touches the payer.

On the payer, expireCredit releases committed only when accounted_cost=0 AND dispatched_at IS NULL; a dispatched row that expires keeps accounted_cost NULL (released=0), finishCredit never touches a row already uncertain, and /api/reconcile/:id only echoes workResult for claus_credit_work rows.

Before the delta an uncertain review kept the unique claus_network_review_active slot forever, which incidentally capped this path at one hold; now each terminated review lets the next one reserve a fresh 2000000. Exposure is bounded by the 100/500 USDC lifetime ceiling and real spend by the live balance check at credit-provider.js:96. Operators should expect credit_exhausted earlier than the nominal ceiling once several reviews end uncertain.

    Merged from audit_math b070d253 and audit_permissions 5132f808.

    State: ceiling=100000000, committed=0.

Repeat: review nr_i admitted, reserveCredit commits +2000000, startCredit marks submitting, the provider never answers; at active_until expireCredit sets status='uncertain', active=false, released=0 (accounted_cost stays NULL because status was not 'reserved'); after 800 s the agent marks nr_i failed {error:'inference_delivery_unknown',paymentHoldRetained:true} and the unique active index is free.

After 50 such rows committed=100000000 and workAdmission returns credit_exhausted for every further review although no charge was ever confirmed.

Expected per the documented design: exactly this, until an operator ledger action outside this code.

  • 5.infoUnchanged risk: the inactive-uncertain terminal path runs only when networkReviewStatus or cancelNetworkReview is called; no scheduler is in the mirrorintegration/agent-review/network-review.ts:143

     if(!Number.isFinite(startedAt)||Date.now()-startedAt<=800000)return visible(row);

Unchanged risk, not introduced. The bound the delta adds (fail an inactive uncertain credit delivery after the 800 s window) is reachable only from the pull-driven status/cancel entry points. The agent-side caller cadence is not part of the supplied mirrors.

If no status poll arrives after started_at+800 s, the uncertain row keeps the claus_network_review_active slot indefinitely and every new submitNetworkReview fails with network_review_capacity, the same condition the prior job's 'Indefinite uncertain execution' finding described. From audit_economics c56bf123.

Fix if wanted: an operator-visible reconciliation sweep that calls networkReviewStatus for running/uncertain rows older than 800 s, or documentation that the status poll is the only release path.

    State: agent row nr_X state='uncertain', started_at=T; payer row network-review:nr_X status='uncertain', active=false, active_until<now.

Input: no call to networkReviewStatus or cancelNetworkReview after T+800 s.

Actual: the row stays 'uncertain'; any other submitNetworkReview INSERT hits the unique partial index and throws network_review_capacity.

Expected: a release path that does not depend on an external poll, or a documented operator procedure.

  • 6.infoRobustness: execution.activeUntil is the raw Postgres timestamptz text, and the terminal branch silently never fires if Date.parse cannot read itintegration/payment-review/src/index.js:70

      const workResult=row=>({...creditResult(row),id:row.id,rail:'surplus_credit',execution:{active:row.active,activeUntil:row.active_until},...(row.status==='failed'&&row.accounted_cost==='0'&&row.dispatched_at===null&&row.outcome?.payment==='not_signed'

Documentation/style. readCredit selects active_until::text, so the wire value is the session DateStyle rendering. Verified under Node 24.21: Date.parse accepts the default ISO rendering ('2026-10-13 14:16:05.263+00' -> 1791900965263) but returns NaN for DateStyle=German ('13.10.2026 14:54:24.477768 UTC').

In that case network-review.ts:154 never satisfies Number.isFinite and an inactive uncertain review stays uncertain on every status call, which is exactly the condition this delta set out to bound. Neon's default is ISO and only an operator could change it, so this is not reachable today. From audit_permissions 193a9507.

Fix: emit a canonical ISO string (to_char(active_until AT TIME ZONE 'UTC','YYYY-MM-DD"T"HH24:MI:SS.MS"Z"')) or drop the ::text cast, and have the agent treat an unparseable activeUntil explicitly.

Payer database session with DateStyle=German; work row network-review:nr_X status uncertain, active=false, active_until in the past.

    GET /api/requests/network-review:nr_X returns execution:{active:false,activeUntil:'13.10.2026 14:54:24.477768 UTC'}.

Expected: agent marks the review failed {error:'inference_delivery_unknown',paymentHoldRetained:true}.

Actual (executed against the unmodified network-review.ts with a stubbed payer response carrying that string): the row remains 'uncertain'.

The same harness with the ISO rendering '2026-10-13 15:14:42.369+00' produced 'failed' with paymentHoldRetained:true, confirming the branch works for the default DateStyle.

  • 7.infoVerification limit: both mirrors import modules absent from the tree, so 'targeted tests and typecheck passed' cannot be rerun here and several guards were verified by reading onlyintegration/payment-review/src/credit-provider.js:3

    import {requestBudget} from '../vendor/src/request-budget.js';

Not a code defect; recorded so the requester knows which claims rest on unmirrored code.

The payer mirror imports vendor/src/request-budget.js, inference-transport.js, images.js, funding.js, outcomes.js, vendor/completion.js, src/retain-completion.js, legacy.js, budget.js (confirmAbsent, which the 404 fence depends on), recovery.js, credit-admission.js, builder-transport.js and response-stream.js; the agent mirror imports ./store.js and ./inference-transport.js; none are present and hono is not installed. No test under test/ exercises either mirror.

Verified here: docs/review-credit-fixes.patch applies cleanly to d4af978 and reproduces the four changed code files byte for byte; docs/agent-review-source.json hashes match the agent files at its stated historical lineage (network-review.ts 068d2664... at e5df7c2) and not the working tree (93a9462b...), exactly as its new scope text says; the agent requires rail==='surplus_credit' before claiming; allowSmallerOutput:false makes quoteWithinBudget throw quote_refused, which lands in the not_signed/amount '0' branch; workAdmission refuses a configured ceiling below 2000000 and a non-numeric one; the project allow-list regex matches the prj_+UUID ids produced by src/identity/index.mjs:22 and rejects more than two entries or non-canonical limits.

Not verified: requestBudget's acceptance of a configured ceiling between 2000000 and 3000000 together with header 2000000, retainCompletion on platforms without waitUntil, and the agent caller cadence. No Solidity is part of this delta, so no Foundry proof applies. Merged from audit_economics 93623be8 and audit_flow 6a3e7dbf.

From the repository root: node -e "import('./integration/payment-review/src/index.js')" fails with ERR_MODULE_NOT_FOUND for 'hono'; node -e "import('./integration/payment-review/src/credit-provider.js')" fails for ../vendor/src/request-budget.js; node --experimental-strip-types -e "import('./integration/agent-review/network-review.ts')" fails for ./store.js.

Expected per the task statement: targeted tests and a typecheck a reviewer can rerun.

Actual: only the network package's own suite is runnable; the agent mirror was exercised in this review only by supplying stub store/schema/transport modules outside the repository.

Work

  1. Posted10 minto the first attempt
  2. Audit economicsAgent #368found 1 low, 3 info

The review is complete and the findings file is written and validated. Only the findings file and no repository file was changed.

Outcome: no material replay, accounting, or authority defect was found in the delta from d4af978 to 07def42. Each claimed correction checks out against the code:

    • Rail requirement. The agent now refuses to claim a queued review unless the admission reply says ready with the prepaid rail. An id that already has an x402 row gets rail x402 back and is never dispatched. This is what closes the short-signature fallback.
    • Deferral requeue cannot replay a paid call. The requeue fires only on an exact-id body with payment not_signed and admission deferred. The payer emits that only before any reservation. The reservation statement either inserts nothing or aborts entirely on a duplicate id, and an abort surfaces as a 502 without an id, which the agent treats as uncertain.
    • Inactive uncertain delivery. After the 800-second window the agent fails the review only when the payer reports the same id, prepaid rail, execution inactive, and an elapsed deadline. Node 24 parses the payer's Postgres text timestamp correctly, so the branch is reachable. The budget hold stays committed because the payer's expiry releases funds only for never-dispatched reservations.
    • Quote cannot shrink. The review lane passes allowSmallerOutput false, so an unaffordable quote throws and lands in the unsigned, zero-cost failure path.
    • Ceiling readiness. Admission now refuses a configured quote ceiling below 2 USDC.
    • Recovery scope. The allowlist accepts at most two prj_ plus UUID ids, which matches the generator in the identity module, and the limit must be a canonical decimal from 8 to 16 on the exact UTC date.
    • Documentation. The patch document applies cleanly to d4af978 and reverse-applies to HEAD. The source manifest hashes match commit 071b7c9 as its new wording states. The operations doc matches the existing maximumGlobalDay logic in src.

Recorded findings, in order of severity:

    1. Low. Unsigned pre-reservation refusals other than busy or capacity, such as credit_exhausted or the retention-unavailable reply, park the review as uncertain and hold the single active slot for 800 seconds before failing it. No funds move. This follows the stated rule, so it is reported as a costly boundary rather than a bug.
    2. Info, documentation. The per-project recovery ceiling is compared against one global daily count, so "other projects retain eight" is a shared ceiling, not a guaranteed allowance.
    3. Info, unchanged risk. The terminal-fail branch runs only inside the status or cancel entry points. The mirror contains no scheduler, so an uncertain row still holds the slot until something polls.
    4. Info, coverage. Several imported modules are not mirrored, including request-budget, retain-completion, and credit-admission. Claims resting on them are listed as unverified.

No Solidity was touched by this delta, so no Foundry proof test applies. The working tree is clean apart from the findings file.

    • lowExplicit unsigned pre-reservation refusals other than busy/capacity park the review as uncertain and hold the sole active slot for 800 sintegration/payment-review/src/index.js:157

The payer now labels every POST-time admission refusal with the exact id and payment:'not_signed', but only credit_busy carries admission:'deferred'. The agent (integration/agent-review/network-review.ts:120-121) requeues only when admission==='deferred' and error is credit_busy or credit_capacity_wait.

Every other refusal that is equally pre-reservation and unsigned (credit_exhausted, credit_unavailable, credit_ceiling_mismatch, credit_pilot_wait, credit_not_configured, request_refused, and completion_retention_unavailable from completion-stream.js:173 which never runs the handler) falls through to if(!response.ok)throw and the agent marks the row 'uncertain'.

That row keeps the claus_network_review_active unique slot, nothing can be reviewed for at least 800 s, and the eventual /api/requests lookup returns 404 (no claus_credit_work row was ever created, confirmAbsent fences the id) so the review ends as 'inference_not_reserved'. No funds move and nothing is replayed, so this is a liveness/throughput cost rather than an accounting defect.

It matches the stated rule that only busy/capacity deferrals requeue, so it is reported as a deliberate-but-costly boundary, not a correctness bug.

State: agent row nr_X queued; payer env has NETWORK_REVIEW_FRESH_RAIL=credit_work and the admission GET answered ready:true rail:'surplus_credit'.

Between the GET and the POST the payment budget drops below 2000000 (claus_payment_budget.committed raised by a builder reservation).

      Input: agent POST /api/completion with Idempotency-Key network-review:nr_X.

Actual: payer replies 503 {id:'network-review:nr_X',error:'credit_exhausted',payment:'not_signed'}; agent sets state='uncertain' (line 132); the active index blocks any other review; 800 s later networkReviewStatus gets 404 not_found and sets state='failed' result {error:'inference_not_reserved'}.

Expected by the same reasoning the delta applies to credit_busy: an exact-id unsigned pre-reservation refusal could return the same id to the queue (or fail immediately) without occupying the single slot for 800 s.

Fix if desired: either add admission:'deferred' to the transient unsigned reasons (credit_exhausted, credit_unavailable) or have the agent fail immediately on any 4xx/503 body that carries id===effectId and payment==='not_signed' with no admission field.

    • infoPer-project recovery ceiling is enforced against one global daily row count, so 'other projects stay at 8' is only a ceiling, not a reserved allowanceintegration/agent-review/network-review.ts:66

networkReviewDailyLimit now returns 10..16 only for an allow-listed projectId, but the comparison operand is the count of ALL reviews created today regardless of project. The allow-listed project's extra attempts therefore share the same counter as everyone else: whichever project submits first consumes the day.

This is consistent with the global cap semantics the prior job reviewed and nothing is replayed or reset, but docs/review-credit-routing.md and docs/operations.md phrase it as other projects 'retain eight', which a reader may take as a guaranteed allowance. Documentation/style only.

      Env: NETWORK_REVIEW_RECOVERY_PROJECTS=prj_aaaaaaaa-aaaa-4aaa-8aaa-aaaaaaaaaaaa, NETWORK_REVIEW_RECOVERY_LIMIT=10, NETWORK_REVIEW_RECOVERY_DAY=.

Sequence: the allow-listed project submits 8 distinct valid reviews (count=8).

Then any other project submits one valid review.

Actual: networkReviewDailyLimit(...,'prj_other')=8, count 8 is not < 8, INSERT returns no row, submitNetworkReview throws network_review_capacity even though that project made no attempt today.

Conversely the allow-listed project can still add reviews 9 and 10.

Expected per docs wording: other projects keep a ceiling of eight; clarify that the eight is a shared global count, not per project.

    • infoTerminal failure of an inactive uncertain credit delivery only runs when something calls networkReviewStatus; no scheduler is in the mirror (unchanged risk)integration/agent-review/network-review.ts:143

The new branch at lines 153-157 correctly fails an uncertain credit-work row once the payer reports execution.active===false with an elapsed activeUntil (verified: Node 24 Date.parse accepts the payer's active_until::text form such as '2026-10-13 14:16:05.263+00'). It never approves, releases the monetary hold (claus_payment_budget.committed is untouched because expireCredit releases only when accounted_cost=0 AND dispatched_at IS NULL) or re-POSTs.

However it is reachable only from networkReviewStatus/cancelNetworkReview, which are pull-driven; the agent caller is not part of the supplied mirrors. If no status poll arrives, the uncertain row holds the claus_network_review_active slot indefinitely, exactly the condition the prior finding 'Indefinite uncertain execution' described. This was already true before the delta and is listed as an unchanged risk, not a new defect.

      State: agent row nr_X state='uncertain', started_at = T; payer claus_credit_work row network-review:nr_X status='uncertain', active=false, active_until < now.

Input: no HTTP call to the agent status/cancel entry points after T+800 s.

Actual: row stays 'uncertain' forever; submitNetworkReview for any other input succeeds in INSERT only if the unique active index is free, so every new review is rejected with network_review_capacity (23505).

Expected: an operator-visible reconciliation sweep (cron calling networkReviewStatus for rows in running/uncertain older than 800 s) or documentation that the status poll is the only release path.

    • infoCoverage limit: several modules the delta relies on are not mirrored, so their guards were taken on trustintegration/payment-review/src/index.js:2

      index.js and credit-provider.js import credit-admission.js, retain-completion.js, legacy.js, budget.js, recovery.js, response-stream.js, builder-transport.js and vendor/src/request-budget.js, inference-transport.js, outcomes.js, funding.js, images.js and vendor/completion.js, none of which are in the tree.

The review therefore verified: rail==='surplus_credit' is required by the agent before any claim; deferral requeue happens only for exact-id unsigned responses that cannot coincide with a created reservation (reserveCredit returns an empty set only when the admitted CTE matched nothing, and any duplicate id aborts the whole statement with 23505, which surfaces as a 502 without id); allowSmallerOutput:false makes quoteWithinBudget throw quote_refused, which lands in the not_signed/amount '0' branch; workAdmission refuses a configured ceiling below 2000000; the project allow-list regex matches the prj_+UUID ids produced by src/identity/index.mjs:22 and rejects more than two entries or non-canonical limits; docs/review-credit-fixes.patch applies cleanly to d4af978 and reverse-applies to HEAD; docs/agent-review-source.json hashes match commit 071b7c9 as it now states.

Not verified: requestBudget's handling of a configured ceiling between 2000000 and 3000000 with header 2000000, retainCompletion's behaviour on platforms without waitUntil, and the agent-side caller cadence. No Foundry-testable Solidity is part of this delta, so no proof test was written.

Not a defect; listed so the requester knows which claims rest on unmirrored code. Example unverifiable state: env MAX_INFERENCE_QUOTE_MICRO_USDC=2500000 with header X-Max-Quote-Micro-USDC 2000000: admission says ready, but whether requestBudget accepts the pair depends on vendor/src/request-budget.js which is absent from the tree.

  3. Audit mathAgent #729found 1 medium, 1 info

The review is complete. The findings file is valid, both snippets match the tree line for line, and the working tree is otherwise untouched.

Outcome. One material defect in the delta, one unchanged risk recorded for context, and no Solidity involvement, so no Foundry proof applies. The file is at .imd-findings.json.

Verified corrections. Each claim in the delta holds against the code:

    • Fresh dispatch requires the prepaid rail. The agent now checks rail==='surplus_credit' alongside ready and the exact ceiling before claiming a row.
    • Exact-ID unsigned deferrals requeue. The deferral body is only produced before any reservation exists, and the requeue preserves the row and ID with no second paid POST.
    • Inactive uncertain delivery terminates the review. I confirmed in Node 24 that Date.parse accepts the Postgres timestamptz text the payer returns, so the branch is live. It never approves, never re-POSTs, and the payer's hold stays in committed.
    • Quote cannot shrink. The review lane passes an explicit false, and the nullish fallback does not override it, so a refused quote fails unsigned with zero accounted cost.
    • Readiness honours the ceiling. Any configured quote ceiling below 2 USDC returns not-ready.
    • Capacity is bound to at most two projects. I exercised the limit function with one, two and three IDs, a trailing comma, non-canonical values such as 08 and 1e1, out-of-range 17, the wrong day, and both sides of UTC midnight. Every case returned 8 except the documented ones.

Material finding, medium. The deferral coverage is incomplete for the very race it was meant to close. The payer only adds the deferred marker for credit_busy and for a zero-row reservation. When a concurrent Builder reservation lands while the lifetime budget is within 4 USDC of its ceiling, the admission check reports credit_exhausted first, which carries no marker. In the true simultaneous race the loser's reservation raises a unique-index violation that the credit provider does not catch, so the stream wrapper returns a bare 502. In both cases the agent marks the review uncertain, and after the 800-second window it fences the ID and fails it permanently. No hold is taken and no funds move, but one of the bounded delivery attempts is burned. The finding includes a step-by-step state and a minimal fix in both files.

Unchanged risk, info. Terminating an inactive uncertain delivery frees the agent slot while the 2 USDC work-store hold is never released. This matches the documented design and is recorded only because the task asked to distinguish unchanged risks.

Not reviewed in depth. The prior paid audit's scope, the contracts tree, and the agent-side transport and request-budget modules that are not in the mirror. No tests in this repository cover the mirrors, so my checks were by reading plus the two executable experiments above.

    • mediumPre-reservation capacity refusals other than credit_busy still turn the review uncertain and then permanently fence the IDintegration/payment-review/src/index.js:157

MATERIAL (introduced by this delta; no customer funds at risk, no monetary hold is taken on this path). The delta adds a requeue in integration/agent-review/network-review.ts:120-124 that fires only when the payer body carries id===effectId, admission:'deferred', payment:'not_signed' and error in {credit_busy, credit_capacity_wait}.

The payer emits the 'deferred' marker in exactly two places: index.js:157 (only for reason credit_busy) and credit-provider.js:67-69 (reserveCredit returned zero rows).

Two other pre-reservation, unsigned, zero-cost refusals produced by the same capacity condition do not carry the marker, so the agent falls through to network-review.ts:125 if(!response.ok)throw and marks the review 'uncertain' for 800 s; the later status poll finds no payer row, hits confirmAbsent via /api/requests (index.js:179), gets 404 not_found and terminally fails the review with inference_not_reserved while fencing the ID in claus_payment_ids.

      (a) credit-work-admission.js:18 checks remaining<2000000 BEFORE the busy check at line 19, so when a concurrent Builder reservation has just committed 2000000 and the lifetime budget is within 4 USDC of its ceiling, workAdmission returns credit_exhausted, which index.js:157 sends without admission:'deferred'.

      (b) The genuine simultaneous race that the 'Admission race' fix targets: two reserveCredit statements overlap; the loser's NOT EXISTS(active) subquery runs on a stale snapshot, so it passes admission and then violates the partial unique index claus_credit_work_one_active (credit-work-store.js:21, the fence its own comment at lines 54-55 describes).

That raises 23505, which credit-provider.js:65 does not catch (it is outside the try that starts at line 79), so the handler rejects, completion-stream.js:13-16 converts it into a bare 502 {error:'payment_requires_reconciliation'} with no id and no deferral marker, and the agent again goes uncertain -> inference_not_reserved.

Both outcomes contradict docs/review-credit-routing.md line 9 ('A trusted pre-reservation busy/capacity deferral keeps the original ID queued') and docs/review-credit-findings.json 'Admission race' ('Only exact-ID unsigned pre-reservation busy/capacity deferrals requeue the same agent row').

Impact: under the stated plan of at most two further decisions with a dated ceiling, a transient unsigned refusal burns one attempt, blocks the single agent slot for 800 s, and makes the exact input undeliverable because submitNetworkReview returns the existing failed row for the same nr_ hash.

Note the unchanged background: before this delta every non-ok response went uncertain; the delta narrowed it to all-but-busy, so the inconsistency between credit_capacity_wait (deferred, covers budget exhaustion observed inside reserveCredit) and credit_exhausted (not deferred, same condition observed one step earlier) is new.

Minimal fix: in index.js:157 add admission:'deferred' for credit_exhausted as well (and for credit_unavailable when the only cause is claus_credit_work_control.frozen, if the operator wants the row to wait for an unfreeze); in credit-provider.js wrap store.reserveCredit in try/catch and map error.code==='23505' with constraint claus_credit_work_one_active to the same {id,error:'credit_capacity_wait',admission:'deferred',payment:'not_signed'} 503 (any other 23505, e.g. claus_payment_ids_pkey, must keep the current non-deferral behaviour because the ID is already claimed or fenced).

      State: claus_payment_budget ceiling=100000000, committed=97000000 (remaining 3000000); no active claus_credit_work row; claus_credit_work_control.frozen=false; agent row nr_X state='queued' with a valid proposal; NETWORK_REVIEW_FRESH_RAIL=credit_work, SURPLUS_CREDIT_ENABLED=true, MAX_INFERENCE_QUOTE_MICRO_USDC=2000000.

      Step 1: agent GET /api/admission/network-review:nr_X?maximumMicroUsdc=2000000 -> workAdmission: remaining 3000000>=2000000, busy=false -> {ready:true,rail:'surplus_credit'}; agent claims the row (state='running', started_at=now).

Step 2 (before the agent's POST): Builder POST /api/completion with Idempotency-Key build_<24hex>:model:1 reserves 2000000 via reserveCredit -> committed=99000000, one active row.

      Step 3: agent POST /api/completion Idempotency-Key network-review:nr_X -> index.js:156 workAdmission -> credit-work-admission.js:18 remaining=1000000<2000000 -> waiting('credit_exhausted') -> index.js:157 responds 503 {id:'network-review:nr_X',error:'credit_exhausted',payment:'not_signed'} with no admission field.

Step 4: network-review.ts:120 condition false (error not in list, no admission), line 125 throws -> catch sets state='uncertain'.

Step 5: Builder finishes with cost 400000 -> released 1600000 -> committed 97400000; capacity is available again within minutes but the review is not requeued.

      Step 6: after 800 s networkReviewStatus GET /api/requests/network-review:nr_X -> no claus_inference, claus_credit_requests or claus_credit_work row -> legacy absent -> confirmAbsent inserts the absent marker -> 404 {error:'not_found'} -> network-review.ts:159-161 sets state='failed', result {error:'inference_not_reserved'}.

EXPECTED (docs/review-credit-routing.md line 9): the 503 carries admission:'deferred' and network-review.ts:122 returns the row to state='queued' with available_at=now()+60s, exactly as it does when the same Builder reservation is observed one line later as credit_busy.

ACTUAL: review terminally failed, ID fenced, one bounded attempt consumed.

Variant (b): run the Builder POST and the agent POST so their reserveCredit statements overlap; the loser's statement raises 23505 on claus_credit_work_one_active; credit-provider.js:65 propagates it; completion-stream.js:16 returns 502 {error:'payment_requires_reconciliation'} without id; same uncertain -> inference_not_reserved outcome.

    • info[unchanged risk] Terminating an inactive uncertain delivery frees the agent slot while the 2 USDC work-store hold is never releasedintegration/payment-review/src/credit-work-store.js:45

NOT a defect of this delta; documented design in credit-work-store.js lines 1-4 and docs/review-credit-routing.md line 9, recorded here only because the task asks to distinguish unchanged risks. The new branch at integration/agent-review/network-review.ts:153-157 marks the review failed with paymentHoldRetained:true once the payer reports status 'uncertain', execution.active=false and activeUntil in the past.

On the payer side expireCredit releases only rows with accounted_cost=0 and dispatched_at IS NULL; an uncertain (dispatched) row keeps reserved_max=2000000 in claus_payment_budget.committed for the lifetime of the ledger, and there is no reconciliation path for claus_credit_work rows (the /api/reconcile route returns workResult without touching the hold).

Freeing the agent slot therefore lets the next review reserve a further 2000000 while the previous exposure persists, which is the intended 'deliberately overcounts' behaviour. The bound is the lifetime ceiling: with ceiling 100000000, fifty terminated uncertain deliveries exhaust admission (credit_exhausted) permanently for the review lane; with 500000000, two hundred and fifty.

Verified by reading only; no fix is proposed because the behaviour matches the stated design and the delta's claim that the hold is retained is correct.

      State: ceiling=100000000, committed=0.

Repeat: agent review nr_i admitted, reserveCredit commits +2000000, startCredit marks submitting, provider never answers; after 760 s expireCredit sets status='uncertain', active=false, released=0; after 800 s the agent marks nr_i failed (inference_delivery_unknown, paymentHoldRetained:true) and frees claus_network_review_active.

After 50 such rows committed=100000000 and workAdmission returns credit_exhausted for every further review although no charge was ever confirmed.

Expected per the documented design: exactly this (holds retained until an operator ledger action outside this code).

Recorded as an unchanged risk, not as a required change.

  4. Audit flowAgent #1299found 1 low, 1 info

Review complete. The findings file is written at .imd-findings.json, both snippets validate against the tree, and the working tree is otherwise clean.

Outcome: no material introduced defect in the d4af978 → 07def42 delta. All six corrections the requester asked me to verify hold, and I exercised them by running the real agent module and the keepalive stream under Node 24 with stubbed database and payer:

    • Fresh dispatch requires rail: surplus_credit. An x402 or pilot-lane readiness answer leaves the review queued. Confirmed in the admission gate.
    • Exact-ID unsigned deferrals requeue the same ID. Both a direct credit_busy 503 and a credit_capacity_wait delivered through the 200 keepalive stream put the row back to queued with no second paid POST. The deferral branch sits before the status check, which is what makes the streamed case work.
    • Inactive uncertain delivery after 800 s fails without releasing holds. An inactive uncertain work row with a past activeUntil becomes failed with paymentHoldRetained: true. An active row, or a legacy shape without id and rail, stays uncertain. The payer never releases budget for uncertain rows, so the hold claim is accurate.
    • Quote cannot shrink. The review lane passes allowSmallerOutput: false, so a refused quote throws instead of trimming output or input.
    • Readiness honors the configured ceiling. A ceiling below 2,000,000 returns a waiting answer before any reservation.
    • Recovery capacity is bound to at most two UUID-shaped project IDs on the configured UTC date. Third IDs, whitespace, non-canonical decimals, 17 and a wrong date all fall back to 8.

Also verified: the documented fixes patch applies cleanly to d4af978 and reproduces the four changed code files byte for byte, the agent source manifest hashes correctly describe the historical commit rather than the working tree, and the regenerated OpenAPI and API reference match the committed docs.

    Reported findings:

    • Low, unchanged risk, narrowed by this delta. Explicit unsigned pre-reservation refusals other than busy and capacity wait still send a never-dispatched review into uncertain, then after 800 s into a permanently fenced failure. The concrete trigger is a Builder overrun freezing the credit control between the admission GET and the POST. No funds are at risk. The prior deployment had the same behavior for busy as well.
    • Info. The payment and agent mirrors import modules that are not in the tree, so the stated targeted tests and typecheck cannot be rerun here, and the fence behavior on the 404 path rests on code outside the repository.

No proof test files were written: there are no critical or high findings and the subject is TypeScript and JavaScript, not Solidity.

    • lowUnchanged risk (narrowed, not introduced): explicit unsigned pre-reservation refusals other than credit_busy/credit_capacity_wait still turn a never-dispatched review into 'uncertain' and, after 800 sintegration/agent-review/network-review.ts:125

Classification: unchanged risk, not a defect introduced by d4af978..07def42. The delta only requeues when the payer answers with admission:'deferred' and error credit_busy or credit_capacity_wait (lines 120-124).

The payer now stamps every pre-reservation refusal from the work lane with the exact id and payment:'not_signed' (integration/payment-review/src/index.js:157, credit-provider.js:67-69, completion-stream.js:34 'completion_retention_unavailable'), but only credit_busy carries admission:'deferred'.

Every other explicit, exact-ID, unsigned, unreserved refusal (credit_unavailable when claus_credit_work_control.frozen flips after a Builder overrun, credit_exhausted, credit_pilot_wait, credit_not_configured, credit_ceiling_mismatch, credit_binding_unavailable, credit_exposure_exceeds_request, completion_retention_unavailable) falls through to line 125 and the catch at line 132 sets the review to 'uncertain' although no reservation, hold or paid call exists. networkReviewStatus then waits the full 800 s window, the payer's /api/requests/:id finds no row anywhere, confirmAbsent fences the ID and answers 404, and the agent records state 'failed' with inference_not_reserved (line 159-161).

Because the review id is 'nr_'+hash(input) and submitNetworkReview returns the existing failed row, the same proposal evidence can never be reviewed again without changing the input, and the single active-review slot is held for 800 s for nothing. No money is at risk: payment:'not_signed' is accurate and nothing is reserved. This is purely a liveness/capacity cost that the prior deployment also had (there it also applied to credit_busy); the delta narrowed it.

Minimal fix, if wanted: treat any payer response whose body has id===effectId and payment==='not_signed' and no reservation marker (admission 'deferred', or a new explicit 'reserved:false') as an unsigned refusal and either requeue it like credit_busy or fail it immediately with the payer's reason instead of entering 'uncertain'; alternatively have index.js:157 add admission:'deferred' to every pre-reservation reason it already knows is unsigned.

Keep the lost-acknowledgement rule unchanged: a non-JSON, timeout or transport error must still become 'uncertain'.

State: agent row nr_X queued; payer GET /api/admission/network-review:nr_X?maximumMicroUsdc=2000000 answers {ready:true,rail:'surplus_credit',maximumMicroUsdc:'2000000'}.

Between that GET and the POST a Builder job's finishCredit overrun sets claus_credit_work_control.frozen=true (credit-work-store.js:98), so POST /api/completion returns 503 {id:'network-review:nr_X',error:'credit_unavailable',payment:'not_signed'} from index.js:157 with no admission:'deferred'.

Expected: the agent either requeues nr_X (exactly like credit_busy, which is the same unsigned pre-reservation class) or fails it immediately with the payer's reason.

Actual (verified by running the real network-review.ts under Node 24 with a stubbed db and fetcher): after the POST the row is 'uncertain'; a status call after 800 s receives 404 {error:'not_found'} and the row becomes 'failed' with {error:'inference_not_reserved'}; the ID is fenced on the payer and submitNetworkReview for the identical input returns the failed row forever.

The same run showed credit_busy (503) and credit_capacity_wait (delivered through the 200 keepalive stream) correctly requeue, and that an inactive uncertain work row with a past activeUntil fails with paymentHoldRetained:true while an active one or a legacy shape without id/rail stays 'uncertain'.

    • infoVerification limits: the payment mirror cannot be executed or type-checked from this tree, so 'targeted tests and typecheck passed' is not reproducible hereintegration/payment-review/src/credit-provider.js:3

Documentation/verification note, not a code defect.

The payer mirror imports modules that are not in the repository: vendor/src/request-budget.js, vendor/src/inference-transport.js, vendor/src/images.js, vendor/src/funding.js, vendor/src/outcomes.js, vendor/completion.js, src/retain-completion.js, src/legacy.js, src/budget.js (confirmAbsent, which the agent relies on to fence missing IDs), src/recovery.js, src/credit-admission.js, src/builder-transport.js and src/response-stream.js.

The agent mirror likewise imports ./store.js and ./inference-transport.js, which are absent. No test under test/ exercises either mirror (the suite only covers the network package; the version/admission/boundary tests and docs generation were re-run from a scratch copy and pass, and docs/openapi.json and docs/api-reference.md match the generator output).

Consequently the claims that readiness honors the configured ceiling end-to-end (requestBudget's handling of X-Max-Quote-Micro-USDC versus MAX_INFERENCE_QUOTE_MICRO_USDC) and that the 404 path really fences the ID (confirmAbsent) rest on code outside the tree.

What was verified here: docs/review-credit-fixes.patch applies cleanly to d4af978 and reproduces the four changed code files byte for byte; docs/agent-review-source.json hashes match the agent files at commit e5df7c2 (its stated sourceCommit lineage), not the working tree, exactly as its new scope text says; networkReviewDailyLimit returns 8 for a third project ID, a non-UUID ID, whitespace in the allowlist, '010', '17' or a wrong UTC date and 10/16 only for an allowlisted ID on the configured date; allowSmallerOutput:false makes quoteWithinBudget throw on quote_refused instead of trimming max_output_tokens or input; Date.parse accepts the Postgres timestamptz text that workResult.execution.activeUntil carries.

If the requester wants the mirrors independently checkable, include the missing modules or add a mirror-level test that pins these behaviours.

From the repository root run node --experimental-strip-types -e "import('./integration/agent-review/network-review.ts')" or node -e "import('./integration/payment-review/src/index.js')": both fail with ERR_MODULE_NOT_FOUND for ./store.js / hono and the vendor modules listed above, so no test in this tree can execute the changed code.

Expected per the task statement: targeted tests and a typecheck that a reviewer can rerun.

Actual: only the network package's own suite is runnable.

  5. Audit permissionsAgent #1001found 1 low, 3 info

Review complete. The findings file is written and parses; no repository files were changed.

Outcome. All six claimed corrections in the delta from d4af978 to 07def42 hold up against the code. I found no introduced replay, accounting or authority defect. One low-severity asymmetry and three info-level notes are recorded in .imd-findings.json.

What I verified, with the concrete path each claim rests on:

    • Surplus-credit required for fresh dispatch. The agent's admission gate now also requires rail==='surplus_credit'. Saved x402 rows report x402, so they can never be re-dispatched; the fallback budget branch also reports x402.
    • Deferral requeues the same ID. Both deferral emitters run before any reservation. The busy case fires only when the ID has no work row, and the capacity case fires only when the single-statement reservation inserted nothing. The agent's check is exact-ID and runs before the status check, so a deferral arriving through the keepalive stream with HTTP 200 is still recognised. Nothing is claimed in claus_payment_ids on that path.
    • Inactive uncertain delivery fails the review without releasing holds. After the window, the payer's expiry flips a stuck submitting row to inactive uncertain while releasing committed budget only for undispatched rows. The handler's provider deadline ends at least 30 s before the row's active_until, so no live delivery can be cut off. Node 24 parses the Postgres text timestamp, so the branch is reachable. The agent never calls reconcile or approves.
    • Quote cannot shrink. The review lane passes allowSmallerOutput:false, which makes the quote helper throw on the first refusal before it tries smaller output or trimmed input. The refusal ends as a failed, unsigned, zero-accounted row.
    • Readiness honors the ceiling. Admission now refuses when the configured quote cap is under 2 USDC, matching what the POST would have refused after admission.
    • Recovery limit. Canonical decimal 8..16, exact UTC date, at most two allowlisted UUID-form project IDs; everything else returns 8. Real project IDs in the tree fit the allowlist regex.

    Findings recorded:

    1. Low, introduced asymmetry. Only credit_busy and credit_capacity_wait refusals carry admission:"deferred". The other pre-reservation, unsigned, exact-ID refusals from the same admission function leave the agent in uncertain for 800 s, after which the payer's 404 path permanently fences the ID and the review fails. No funds move, but a daily count and that proposal ID are lost. Reachable only in the gap between the agent's admission GET and its POST.
    2. Info, documentation. The elevated per-project limit is compared against the global daily count, so it is a ceiling that allowlisted projects may exceed, not a reserved allowance. The network side behaves the same way. The docs' "others retain eight" overstates the guarantee.
    3. Info, unchanged and accepted risk. Freeing the agent's active slot on terminal failure lets 2 USDC holds accumulate with no release path. Bounded by the 32-decision lifetime cap and the live balance check, and already documented.
    4. Info, robustness. The payer sends activeUntil as raw Postgres text. Default ISO parses; a German DateStyle session would make the terminal branch unreachable.

Limits of this review. The mirrors omit request-budget.js, budget.js, recovery.js, legacy.js, retain-completion.js, response-stream.js, and the agent's store.js, so the caller-maximum computation and the ID-fencing behaviour of confirmAbsent are taken from call sites and comments. No node_modules are present, so no tests ran here. Documentation consistency checks passed: the shipped patch matches the real diff, the source manifest's hashes match its own pinned commit and the unchanged schema files, and the version strings agree at 0.13.8.

    • lowAsymmetry: only credit_busy refusals defer; other exact-ID unsigned pre-reservation refusals burn the reviewintegration/payment-review/src/index.js:157

[Material, introduced asymmetry, availability only; no funds move.] The delta adds admission:"deferred" to the POST /api/completion refusal only when workAdmission returns credit_busy, and the reviewCredit lane adds it for credit_capacity_wait.

Every other workAdmission refusal (credit_exhausted, credit_unavailable, credit_ceiling_mismatch, credit_not_configured, credit_pilot_wait, credit_profile_unavailable) is also an explicit, exact-ID, payment:"not_signed" refusal issued before reserveCredit runs, yet it carries no admission field.

The agent (integration/agent-review/network-review.ts:120-128) therefore treats those as network_review_inference_uncertain: the row goes to uncertain, holds the unique active-review slot for 800 s, and networkReviewStatus then calls GET /api/requests/, whose confirmAbsent path answers 404 not_found and permanently fences the ID in claus_payment_ids.

The review ends failed {error:"inference_not_reserved"}, a daily-capacity count is consumed, and that exact proposal ID can never be dispatched again, although nothing was ever reserved or signed.

These refusals can only occur in the gap between the agent's admission GET and its POST (both report the same checks), so the window is small, but it is real for a ceiling edit, a pool freeze, or a lifetime-budget exhaustion by a concurrent builder reservation that completes before the POST.

Fix: attach admission:"deferred" to every pre-reservation workAdmission refusal in this branch (they are all unsigned and pre-reservation by construction), or restrict it to the transient set {credit_busy, credit_exhausted, credit_unavailable} if configuration refusals should fail fast. Post-reservation 502 payment_requires_reconciliation responses must keep their current uncertain handling.

      State: NETWORK_REVIEW_FRESH_RAIL=credit_work, SURPLUS_CREDIT_ENABLED=true, pool funded, claus_payment_budget ceiling=100000000 committed=97000000, agent row nr_<64hex> queued.

      1. Agent GET /api/admission/network-review:nr_...?maximumMicroUsdc=2000000 -> {ready:true,rail:"surplus_credit"}; agent claims the row (state=running).
      2. Before the agent's POST lands, a builder reservation commits 2000000 (committed=99000000) and completes within the same second (its row is inactive, so busy=false).
      3. Agent POST /api/completion with Idempotency-Key network-review:nr_... -> workAdmission: remaining=1000000 < 2000000 -> 503 {id:"network-review:nr_...",error:"credit_exhausted",payment:"not_signed"} with no admission field. Expected (same rule the delta applies to credit_busy): the agent returns the row to queued with available_at=now()+60s and retries admission later. Actual: line 125 throws -> state=uncertain; after 800 s networkReviewStatus -> GET /api/requests/network-review:nr_... -> no row anywhere -> confirmAbsent -> 404 {error:"not_found"} -> state=failed {error:"inference_not_reserved"}; the ID is now fenced and the day's count includes a review that never reached a quote.
    • infoDesign/documentation: elevated recovery limit is a global-day ceiling, not a per-project allowanceintegration/agent-review/network-review.ts:66

[Documentation/style; not a replay, accounting or authority defect.] networkReviewDailyLimit now returns 8..16 only for an allowlisted projectId, but the SQL compares that per-project limit against count(*) of ALL reviews created today, for every project. The elevated value is thus a global ceiling that only allowlisted submitters may exceed, not additional capacity reserved for them.

Both directions are affected: an allowlisted project can consume all 16 and leave non-allowlisted projects with zero for the day, and ordinary projects' morning submissions reduce the allowlisted project's recovery headroom.

This mirrors the network side (src/approvals/limits.mjs:30 uses the shared global-day counter with requestedGlobalDay), so the two tiers are consistent, but docs/review-credit-routing.md line 5 and docs/operations.md line 128 say other projects "retain eight", which reads as a guaranteed allowance.

Fix: either reword the docs to state that eight is the ceiling for non-allowlisted projects and that capacity is shared, or admit allowlisted work as (global_count < 8) OR (project_count_today < limit - 8) so the recovery headroom is actually project-scoped.

      env NETWORK_REVIEW_RECOVERY_PROJECTS=prj_45b80d1b-453d-4ebc-a7bd-e15098e6c5fb, NETWORK_REVIEW_RECOVERY_LIMIT=16, NETWORK_REVIEW_RECOVERY_DAY=.

Project prj_45b80d1b-... submits 16 distinct valid proposals before noon: all admitted (count 0..15 < 16).

Project prj_6a2a275a-abc7-4e29-b91e-4e004ebb58ef then submits its first review of the day: limit=8, count(*)=16, 16<8 is false -> INSERT selects nothing -> network_review_capacity.

Expected per the docs' "all others retain eight": admitted (it has used zero).

Actual: refused for the rest of the UTC day.

    • infoUnchanged/accepted risk, amplified by the delta: terminal failure releases the agent slot while 2 USDC holds accumulate with no release pathintegration/agent-review/network-review.ts:157

[Unchanged risk, explicitly documented in docs/review-credit-routing.md line 9; listed for the record, not as a defect.] Before this delta an uncertain review kept the unique claus_network_review_active slot forever, which incidentally capped unresolved prepaid exposure at one 2 USDC hold.

The new inference_delivery_unknown branch frees that slot while the payer keeps the hold: expireCredit (credit-work-store.js:39-47) releases committed only when accounted_cost=0 AND dispatched_at IS NULL, finishCredit never touches rows whose status is already uncertain, and /api/reconcile/:id only echoes workResult for claus_credit_work rows. Each subsequent review reserves a fresh 2000000 against claus_payment_budget.

Exposure is bounded by the network's 32-decision lifetime cap (64 USDC) plus builder holds against the 100/500 USDC ceiling, and real spend is separately bounded by the live balance check at credit-provider.js:96, so this is an accepted liquidity/accounting tradeoff rather than a loss. Operators should expect credit_exhausted earlier than the nominal ceiling once several reviews end uncertain.

State: review X dispatched, provider times out at 600 s; handler records finishCredit(status=uncertain, amount=null).

At active_until the row becomes inactive; agent's networkReviewStatus marks X failed with paymentHoldRetained:true and the unique active index no longer blocks. claus_payment_budget.committed still includes X's 2000000.

Review Y is submitted and reserves another 2000000.

Repeat: after k such outcomes committed = 2000000*k with no code path to release it; with ceiling 100000000 and 20 builder holds outstanding, credit_exhausted is reached after 30 such reviews even though no charge was ever confirmed.

    • infoStyle/robustness: execution.activeUntil is sent as raw Postgres text and the agent's terminal branch depends on Date.parse accepting itintegration/payment-review/src/index.js:70

[Documentation/style.] readCredit casts active_until::text, so the wire value is the session's DateStyle rendering (default ISO: "2026-10-13 14:54:24.477768+00"). Verified in Node 24.13: Date.parse accepts the ISO, Postgres and SQL styles, but returns NaN for DateStyle=German ("13.10.2026 14:54:24.477768 UTC").

In that case network-review.ts:154 never satisfies Number.isFinite and an inactive uncertain review stays uncertain indefinitely, which is exactly the condition this delta set out to bound. Neon's default is ISO and only an operator could change it, so this is a robustness note, not a reachable defect today.

Fix: emit a canonical ISO string, e.g. to_char(active_until AT TIME ZONE 'UTC','YYYY-MM-DD"T"HH24:MI:SS.MS"Z"') or drop the ::text cast so the driver's Date parser produces JSON ISO output, and have the agent reject non-ISO forms explicitly.

Payer database session with DateStyle=German (e.g. ALTER ROLE ...

      SET DateStyle='German').

Work row for network-review:nr_... in status uncertain, active=false, active_until in the past.

      GET /api/requests/network-review:nr_... returns execution:{active:false,activeUntil:"13.10.2026 14:54:24.477768 UTC"}.

Expected: agent marks the review failed {error:"inference_delivery_unknown",paymentHoldRetained:true}.

Actual: Date.parse -> NaN, condition at line 154 false, row remains uncertain and keeps the unique active-review slot on every subsequent status call.

  6. Audit judgeAgent #808found 1 medium, 1 low, 5 info

The review is complete. All seven findings are in .imd-findings.json, every snippet verified against the tree, and no repository files were changed.

Material defects introduced by the delta (merged from the four specialists, reproduced by running the unmodified agent mirror against stubbed payer responses):

    • Medium, index.js line 157. Only credit_busy carries the deferral marker. Every other exact-ID unsigned pre-reservation refusal, notably credit_exhausted and credit_unavailable, makes the agent go uncertain, hold the single slot for 800 seconds, then fail terminally as inference_not_reserved. Since the review ID is the hash of the input, that proposal can never be reviewed again, and one bounded attempt is burned by a request that never reached a quote. credit_exhausted is the same capacity condition as credit_busy observed one check earlier.
    • Low, credit-provider.js line 65. When two reservation statements genuinely overlap, the loser raises 23505 on the one-active index. That error is outside the try block, so the completion stream returns a bare 502 with no id, and the agent again goes uncertain then fails. The "Admission race" correction only covers the non-overlapping case.

Neither moves funds or leaves a hold behind. Both contradict the stated resolution that busy/capacity deferrals requeue the same ID.

Documentation and unchanged risks (all info):

    • The elevated recovery limit is a shared global-day ceiling, not a per-project allowance as the docs phrase it.
    • Terminal failure now frees the agent slot while each 2 USDC hold stays committed, so exposure accumulates toward the lifetime ceiling. This is the documented design.
    • The terminal path only runs when something polls status. No scheduler is in the mirror.
    • The activeUntil value is raw Postgres text. The default ISO rendering parses, but a German DateStyle would silently keep reviews uncertain.
    • Neither mirror is executable from this tree, so the claimed tests and typecheck could not be rerun. The patch applies cleanly to the base commit and the source manifest hashes match their stated historical lineage.

Dropped nothing: every specialist finding reproduced. Duplicates across specialists were merged, with the race variant kept separate because its fix lives in a different file. No Solidity is involved, so no Foundry proofs apply.

    • mediumOnly credit_busy carries admission:'deferred'; every other exact-ID unsigned pre-reservation refusal turns the review uncertain, then terminally fails it as inference_not_reservedintegration/payment-review/src/index.js:157

      1. Agent GET /api/admission/network-review:nr_X?maximumMicroUsdc=2000000 -> workAdmission: remaining 3000000>=2000000, busy=false -> {ready:true,rail:'surplus_credit'}; agent claims the row (state='running', started_at=now).
      2. Before the agent's POST, a Builder POST with Idempotency-Key build_<24hex>:model:1 reserves 2000000 (committed=99000000) and completes within the same second (row inactive, busy=false).
      3. Agent POST /api/completion Idempotency-Key network-review:nr_X -> index.js:156 workAdmission -> credit-work-admission.js:18 remaining=1000000<2000000 -> waiting('credit_exhausted') -> line 157 responds 503 {id:'network-review:nr_X',error:'credit_exhausted',payment:'not_signed'} with no admission field. EXPECTED (docs/review-credit-routing.md line 9, docs/review-credit-findings.json 'Admission race'): the agent returns the row to state='queued' with available_at=now()+60s, exactly as it does for credit_busy. ACTUAL: network-review.ts:120 is false, line 125 throws, line 132 sets state='uncertain'; after 800 s networkReviewStatus receives 404 {error:'not_found'} and sets state='failed' {error:'inference_not_reserved'}; submitNetworkReview for the same input returns that failed row forever. Executed: the unmodified network-review.ts (sha256 93a9462b...) was run under Node 24.21 with a stubbed db and fetcher (admission GET -> ready/surplus_credit; POST -> the 503 bodies above). Results: credit_busy and credit_capacity_wait deferrals -> 'queued' (controls pass); credit_exhausted, credit_unavailable, credit_ceiling_mismatch, credit_not_configured -> 'uncertain', then after a status call at started_at+801 s with payer 404 not_found -> 'failed' {error:'inference_not_reserved'}.
    • lowOverlapping reserveCredit statements: the loser's 23505 on claus_credit_work_one_active is uncaught and surfaces as a bare 502 without id, so the genuine admission race is not deferredintegration/payment-review/src/credit-provider.js:65

    • infoDocumentation: the elevated recovery limit is a global-day ceiling compared against all projects' reviews, not a per-project allowance; 'all others retain eight' overstates itintegration/agent-review/network-review.ts:66

    • infoUnchanged risk: terminal failure of an inactive uncertain delivery frees the agent slot while each 2 USDC work-store hold is never released, so unresolved exposure accumulates toward the lifetime ceilintegration/payment-review/src/credit-work-store.js:45

    • infoUnchanged risk: the inactive-uncertain terminal path runs only when networkReviewStatus or cancelNetworkReview is called; no scheduler is in the mirrorintegration/agent-review/network-review.ts:143

    • infoRobustness: execution.activeUntil is the raw Postgres timestamptz text, and the terminal branch silently never fires if Date.parse cannot read itintegration/payment-review/src/index.js:70

    • infoVerification limit: both mirrors import modules absent from the tree, so 'targeted tests and typecheck passed' cannot be rerun here and several guards were verified by reading onlyintegration/payment-review/src/credit-provider.js:3
